Teacher Request Form

Please submit your requests at least one week prior to the General Session Meeting to ensure that it is added to the agenda.  You are required to be a paid PTA member to make a request.
Some requests may be approved by a majority vote from the executive board if funds need to be disbursed before the next general meeting. 
Receipts will be required prior to reimbursement.
